All about me:     

Hannah is a beautiful little girl with a smile that lights up her face.  She has long blond hair and blue eyes and is of average height and build for her age.  She loves to play princesses, Barbie and unicorns as well as enjoying craft activities and more active play on her bike/scooter/swimming.

Tom is a gorgeous little boy with a cheeky smile and personality to match. He has short fair hair and blue eyes and is of average height and build for his age.  He loves to be active and on the go, he likes to play with superheroes and dinosaurs as well as enjoying craft activities and more active play on his bike/scooter/swimming.

Education and Health: 

Hannah and Tom were born early and both spent some time in special care due to respiratory and feeding issues.  Currently Hannah and Tom are well with no additional health needs identified. Their birth parents are both known to misuse drugs/alcohol and experience poor mental health.  Hannah and Tom are both currently accessing school/nursery.  They are thriving in their prospective school/nursery environments and making good progress in all areas.  Birth parents have no recorded additional learning needs.

Reasons for Coming into Care: 

Hannah and Tom were discharged from hospital into the care of their birth mum. They remained in her care subject to both child protection or child in need plans.  Sadly, in August 2024 the children were found in a very neglected state and needed alternative carers to be found for them.  They initially went to stay with a family member but this was unable to be sustained so they moved to live with local authority foster carers. Hannah and Tom have remained with the same foster carers since September 2024 and are thriving with the consistent care they are now receiving.  They have a very strong brother-sister bond.

Hannah and Tom have 4 older half brothers and sisters who do not live with birth parents due to historical and long-standing issues of neglect, domestic abuse and parental substance misuse.

Type of Family Needed: 

Hannah and Tom’s prospective adopters would need to be accepting of their past experiences and have a good understanding of the life-long impact of adoption.  Prospective adopters will need to be resilient and nurturing and be able to provide the children with a clear sense of belonging both now and in the future. They need to accept a level of uncertainty about Hannah and Tom’s future development given their lived experiences.  Adopters will need to be able to promote an ongoing staying in touch plan.

Hannah and Tom require a one or two parent family of any gender.  They can live with pets.

Staying in Touch Plan: 

Birth mum and dad’s future staying in touch plan has been assessed as indirect only.  A birth parent meeting with prospective adopters is to be assessed closer to the time and will be dependent on the status of birth parents’ mental health.

It is anticipated maternal half-brothers and sisters’ future staying in touch arrangements will be both direct and indirect.  It is also hoped an adopter/half-sibling meeting will be able to take place.
